3 Easy Steps to get your Medical Green Card

1. Make An Appointment with a Registered Physician 

If you already have a regular family physician, the easiest way to start may be to call your physician’s office and explain that you wish to submit an application for a Montana Medical Green Card. An office visit may or may not be required – it varies by clinic & physician.

 

Once your physician completes the paperwork, you can pick it up and proceed with the application process

                    – OR – 

If you do not have a regular family physician or you are unable to obtain a Physician’s Statement from your family doctor, you can make an appointment with our recommended  

physician at this local clinic by 

2. Attend your appointment 

3. Go to Complia  and follow the instructions:

You're all set. An email by Montana DPHHS will be sent to notify you of your application confirmation.   

They will present you with your pamphlet, from there you can choose to submit your paperwork online with the step by step process in the back of your paperwork or bring it to us and we would be happy to help you!

Here are some of the debilitating conditions for which Medical Cannabis is recommended by doctors:

Cancer, glaucoma, positive status for human immunodeficiency virus, or acquired immune deficiency syndrome when the condition or disease results in symptoms that seriously and adversely affect the patient’s health status;

Severe chronic pain that is persistent pain of severe intensity that significantly interferes with daily activities as documented by the patient’s treating physician

Intractable nausea or vomiting;

Multiple sclerosis;

Crohn’s disease;

Epilepsy or an intractable seizure disorder;

Painful peripheral neuropathy;

A central nervous system disorder resulting in chronic, painful spasticity or muscle spasms;

Admittance into hospice care in accordance with rules adopted by the department;

Post-Traumatic Stress Disorder
